Reset PIN Instructions

In order to change your VISA debit card personal identification number (PIN), please follow the steps listed below. These steps were designed to ensure the safety of your card and the privacy of your new PIN. If you have any questions, please ask. We will be happy to assist you.

Step #1 Completing the VISA debit card PIN Request Form
Print and complete the information on both the upper and lower sections of the VISA debit card PIN Request Form

Step #2 Mailing
Mail your VISA debit card along with the top portion of the VISA debit card PIN Request Form in an envelope addressed to:

Mail the lower portion of the VISA debit card PIN Request Form in an envelope addressed to:

Customer Service Department
P.O. Box 7933
Madison, WI 53791

Please note:

• AnchorBank will not be responsible for VISA debit cards lost in the mail.
• If a VISA debit card and a PIN request are sent in the same envelope, AnchorBank will destroy the card.
